What are the causes of incidents and why should they be investigated?

An accident investigation is a Injuries, illnesses, damage to health, and fatalities that resulted or could have resulted from any incident as defined by the Occupational Safety and Health Administration.
"Accident" is also commonly used to describe situations in which events occur unpredictably that result in injury or property damage (or not). There is a tendency to mix up the two terms from time to time. There are instances of injury or damage that may occur in the future, but have not already occurred. An event that could have cause harm but did not is called a "near miss" or "dangerous occurrence."
Those involved in incident investigation teams, supervisors, health and safety committees, or employers can use the information below as a guide. In order to prevent the recurrence of incidents, investigators should determine the root cause of those incidents. In this case, it is not about finding fault, but rather about identifying facts that could lead to a solution. Identifying the problem's root cause is always important.
